ABSTRACT

Objective:To investigate the clinical efficacy of anterior approach combined with blocking plates and screws in the management of acetabular fracture involving the quadrilateral area.Methods:A retrospective case series analysis was performed for 16 patients with acetabular fracture involving the quadrilateral area admitted to First and Second Affiliated Hospital of Guangxi Medical University from January 2017 to January 2019. There were 12 males and 4 females,with the age of 21-66 years[(45.3±10.6)years]. According to Letournel-Judet classification,there were 9 patients with bi-column fracture,6 with anterior and posterior traverse fracture and 1 with anterior column fracture. A total of 9 patients were operated via the ilioinguinal approach and 7 via the lateral-rectus approach. Reduction and fixation of the pelvis and acetabulum were performed,using 3.5 mm cortical bone screws or plates to block the internal displacement of fracture in the quadrilateral body. The incision length,operation time and intraoperative blood loss were recorded. The quality of fracture reduction was assessed according to the Matta reduction criteria at postoperative 2 days and hip function by the modified Merle D'Aubigne-Postel score at postoperative 3 months and 12 months. Postoperative complications were observed.Results:All patients were followed up for 13-24 months[(16.1±2.9)months]. The ilioinguinal approach and lateral-rectus approach showed surgical incision of 12-26 cm[(18.6±4.0)cm]and 8-15 cm[(10.7±2.3)cm],respectively. The operation time was 107-215 minutes[(159.2±27.8)minutes]and the intraoperative blood loss was 200-2,300 ml[(853.1±489.7)ml]. According to Matta reduction criteria,the results were excellent in 9 patients and good in 7. Three months after operation,the modified Merle D'Aubigne-Postel score was 11-18 points[(15.2±2.2)points],which showed the results were excellent in 4 patients,good in 7,fair in 4 and poor in 1,with the excellent and good rate of 69%. Twelve months after operation,the modified Merle D'Aubigne-Postel score was 13-18 points[(16.9±1.4)points],which showed the results were excellent in 7 patients,good in 8 and fair in 1,with the excellent and good rate of 94%. The liquefaction of post-surgical incision was seen in a patient,bladder injury in a patient,lateral femoral cutaneous nerve injury in a patient,and heterotopic ossification in a patient. There was no loosening or breakage of the internal fixation.Conclusion:For acetabular fracture involving the quadrilateral area,anterior approach combined with blocking plates and screws can prevent the displacement of quadrilateral fracture and attain satisfactory reductiongood hip function recovery and few complications.

ABSTRACT

The present advantage and disadvantage or problems of basic medical sciences elective courses in college and university are analyzed and indicated.Meanwhile,the experience of teaching optional course of the clinical applied anatomy of heart has been presented,including reforming teaching method,combining the anatomical theory with the clinical specialty,especially boosting the interesting in the course and medical research,encouraging students to do more dissection and culturing their competence in learning and following closely the trends of medical science and technology development.The new teaching style received a better teaching effect and improved the teaching quality. The significance that the teachers should pay more attention to practice training in their teaching students of optional courses has been positively emphasized.

ABSTRACT

To explore primarily the reconstruction of biological tissue engineered blood vessel in vitro, the decellularised scaffolds were obtained from swine common carotid arteries by enzyme digestion, and vascular smooth muscle cells (VSMCs) were isolated from canine thoracic aorta and subcultured and purified. The VSMCs were seeded into the inner surface of scaffolds and they were cocultured in vitro for 4 weeks. Histological staining and transmission electron microscopy were used to observe the growth of canine VSMCs on swine decellularized arterial scaffolds. The results showed the seeded VSMCs grew well on the decellularized scaffold throughout the duration of 4 weeks, which suggests that the method in this article is practicable for reconstructing complete biological tissue engineering blood vessel.

ABSTRACT

To explore the biological effects of shear stress on intact artery and the change of growth factor during stress-induced vascular remodeling, we established an artery organ-cultured system under stress in vitro, and the common carotid arteries of pigs were cultured under shear stress of 20, 5 and 0 dyn/cm2. PDGF-A synthesis of vascular smooth muscle cells (VSMCs) cultured for 1, 4 and 7 days were studied by immunohistochemical and computer image processing methods, and PDGF-B secretion of endothelial cells (ECs) cultured within 12 h were studied by ELISA. Results showed that PDGF-B increased obviously under shear stress of 5 dyn/cm2, and reached the highest point at about 3 h; PDGF-A synthesis also obviously increased under low shear stress in 7 days. Increasing of PDGF synthesis promotes phenotype switch and proliferation of VSMC. It may have important influence on artery remodeling under low shear stress.

ABSTRACT

Beijing has been one of the epicenters attacked most severely by the SARS-CoV (severe acute respiratory syndrome-associated coronavirus) since the first patient was diagnosed in one of the city's hospitals. We now report complete genome sequences of the BJ Group, including four isolates (Isolates BJ01, BJ02, BJ03, and BJ04) of the SARS-CoV. It is remarkable that all members of the BJ Group share a common haplotype, consisting of seven loci that differentiate the group from other isolates published to date. Among 42 substitutions uniquely identified from the BJ group, 32 are non-synonymous changes at the amino acid level. Rooted phylogenetic trees, proposed on the basis of haplotypes and other sequence variations of SARS-CoV isolates from Canada, USA, Singapore, and China, gave rise to different paradigms but positioned the BJ Group, together with the newly discovered GD01 (GD-Ins29) in the same clade, followed by the H-U Group (from Hong Kong to USA) and the H-T Group (from Hong Kong to Toronto), leaving the SP Group (Singapore) more distant. This result appears to suggest a possible transmission path from Guangdong to Beijing/Hong Kong, then to other countries and regions.

ABSTRACT

Objective To investigate effects of low shear stress on atherosclerosis,the development of atherosclerosis and expression of ?actin and cMyc proteins were studied in VSMC of common carotid artery under low shear stress. Methods We established the atherosclerotic model of left common carotid artery that presented low shear stress by the operation of ligating the left external carotid artery in rabbits.The histomorphology of the carotid arteries was examined by light microscopy.The expression of ?actin and cMyc was studied by immunohistochemistry. Results The typical atherosclerosis was observed and the atherosclerotic lesion was obviously accelerated under low shear stress.The expression of ?actin was downregulated and the phenotype of VSMC converted from contractile phenotype into synthetic phenotype,and expression of cMyc increased in VSMC of the artery under low shear stress.Conclusion This new model of atherosclerosis under low shear stress was successfully established,which provides a new tool for studying the effects of low shear stress on atherosclerosis.The results show that the atherosclerotic lesion is obviously accelerated and VSMC tends to proliferate in response to low shear stress.

ABSTRACT

The construction and amount of fibronectin (Fn), laminin (Ln) and collagen type IV (ColIV) in the extracellular matrix of endothelial cells (EC) co-cultured with vascular smooth muscle cells (VSMC) under shear stress were studied by immuno-fluoro-cytochemistry, laser confocal scanning microscopy and image analysis methods to detect the effect of shear stress on adhesion of EC for vascular tissue engineering. One group of EC was maintained under static conditions as a control. In the control, both Fn and Ln exhibited as a granular pattern in perinuclear area and a fibrillar pattern localized underneath EC, and Col IV exhibited mainly as the granular in the perinuclear area, but the fibrillar was fewer. On exposure of EC to shear stress in physiological range, Fn grouped into fibril tracts, and there was a tendency for some of these tracts of fibrils to align with direction of shear stress. Ln and Col IV also grouped into fibril tracts, which, in contrast to Fn, were randomly oriented, Simultaneously, all contents of the extracellular matrix had different quantitative alterations. These results suggest that the capability of adhesion of EC co-cultured with VSMC may be strengthened under shear stress.

ABSTRACT

Objective The effect of vascular smooth muscle cells (VSMCs) on the PDFG\|B mRNA expression of Co\|cultured endothelial cells (ECs) under shear stress had been studied in order to provide some experimental data for preventing the development of restenosis resulted from VSMCs proliferation in the vascular graft. Methods The level of PDGF\|B mRNA of ECs cultured alone and Co\|cultured with VSMCs exposed to shear stress were investigated by the methods of in situ hybridization and image analysis. Both controls of ECs cultured alone and Co\|cultured with VSMCs were maintained under static conditions. Results In static condition, the content of PDGF\|B mRNA expression of Co\|cultured ECs was lowered in comparison with ECs alone\|cultured. Under shear stress, PDGF\|B mRNA expression of Co\|cultured ECs was increased in transient manner at 1hour after the onset of shear stress and then returned to the level lower than the static level of Co\|cultured ECs at 6hours after being exposed to shear stress. The transient increasing time of PDGF\|B mRNA expression of Co\|cultured ECs reached a maximum earlier than that of ECs cultured alone.Conclusion Under shear stress, PDGF\|B mRNA expression of ECs Co\|cultured with VSMCs is lowered, which may be favor of inhibiting VSMCs proliferation.
